The NCI investigator registration process is centralized in the Clinical Trials Education Office (CTEO) and the Quality Assurance Office for Clinical Trials (QACT) for DF/HCC investigators. Registering a New Investigator: To register with the NCI a Statement of Investigator Form (FDA Form 1572), Supplemental Investigator Form (IDF), Financial Disclosure Form, and CV must be sent to the CTEO. Annual Renewal: The renewal process occurs between the months of June-August of each year. All DF/HCC investigators must renew their registrations at this time regardless of when they last registered. Renewal consists of submitting a Statement of Investigator Form (FDA Form 1572), Supplemental Investigator Form (IDF), Financial Disclosure Form, and CV to the CTEO. These forms are distributed to the Investigators in a bright red envelope in the beginning of June. Each red packet contains the necessary forms as well as instructions on how to complete the forms and a deadline of form submission.
