The process for attestation forms has been updated. Please note this is an operational form to assist the study team in tracking that COI is appropriately reported to the IRB per study, as applicable. All attestation forms will route back to the study team and will no longer route to the IRB. The study contacts will receive an email when the Investigator completes the form.
There are three types of emails you may receive:
- Completed and no conflict of interest. When this is received, the study team should update the key study personnel from Pending –Investigator to Sub-Investigator
- Blank: Create a new attestation form and re-send to the Investigator.
- Important Reminder: The system will default to include you as the submitter in the sign-off. You must uncheck yourself and ensure only the investigator is checked. If you route it to yourself and sign-off, this locks the form and the investigator will not be able to indicate if they have an outside interest to disclose.
- Conflict of interest: Reach out to the Investigator and collect their conflict of interest form. Once received, you will need to submit an Administrative Modification. See previous communication here.
