InForm Information

InForm is an Electronic Data Capture (EDC) application that manages, stores, and retrieves data generated from clinical trials. It captures data into core electronic case report forms for DF/HCC disease centers and allows for remote access to studies implemented within the application. In addition, InForm allows for edit checks to ensure data quality, and improve turnaround time for study analysis and reporting.

For non-PCC issues, submit an IS Service Desk ticket and indicate “InForm – DFCI” in the "Application In Use" field.
